  QUALIFYING CRITERIA
Is the Gestational age ≥ 36 weeks
Is the baby ≤ 6 hours from birth
  (A) Evidence of peripartum hypoxia-ischemia
Apgar ≤ 5 @ 10 minutes
Cord or first hour arterial gas base excess ≤ -16 mmol/L
IPPV ≥ 10 minutes
Cord or first hour arterial gas pH ≤ 7
  (B) Evidence of Moderate-to-Severe Encephalopathy (clinical)
Decreased level of consciousness
Decreased or no spontaneous activity
Abnormal posture
Decreased Tone
Weakness in any primitive reflexes (Moro or suck)
Constricted or variable pupils
Bradycardia or variable heart rate
Shallow breathing or apnea
Seizure
